5. "Buckethead" - This is the nickname of Nova, The Human Rocket.

That is because, for a costume, he wore a helmet that looked like a bucket.

Don't forget "Bic Head", which Ben used to call Johnny! That's non-obvious, and even has a little product placement in there!

And in that vein, Juggernaut was referred to as "Edsel Head" a couple times. (and the fact that I know what that means really shows my age)

How about Farnoq Dahn from the "War and Pieces" storyline from PAD's Hulk? Strong Guy christened him "Stupid-Head." I know it's obscure, but it's the best I can do.

Perhaps the Iron Man nickname of "shellhead" didn't mean that his head was shpaed like a shell, but the fact that his helmet covers his whole head and face LIKE a shell. Thus, "shellhead." Seems rather obvious, to me. ;)

I think you we on the money with Marvels' heroes not being the most creative when it comes to nicknames. Heh.

Hmm. A more pertinent question might be why it's only Marvel that does these "head" nicknames.

Come to think of it, I don't know that DC are traditionally that big on giving their characters nicknames at all.
